6th grade immunizations

The Pennsylvania Department of Health requires all students entering 7th grade to have proof of varicella immunization or chickenpox and Hepatitis B.   Those students who do not have a record of these immunizations in their school health record will receive forms in their report card packets.  Phone calls will be made to the parents of those students that still do not meet the minimum requirements the first week of May.   If your child already has completed this requirement please fill in the form with the dates and return it to me.  If your child does not have these injections please make arrangements with either your family physician or the State Health Clinic as soon as possible.  Even tho the next school year may seem very very far away keep in mind Hepatitis B is a series of 3 injections which takes approximately 6 months to complete. Those students who do not meet the minimum requirements will be excluded from entering 7th grade until the requirement is metIf you have any questions or concerns please email or call me.

Height and Weight


All sixth grade students are weighed and measured during phys ed class in the first week of school.  Based on this information you will be notified of your child's BMI in a letter with an explaination of it's meaning in their report card envelope usually third marking period.  

Physical Exams


A physical exam is required for all 6th graders in the state of Pennsylvania. This exam can be done 1 year prior to the start of 6th grade and anytime during 6th grade.

 This can be done in one of two different ways: 

1.) School Physical form filled out by your private physician and returned to the school by October 31.  If your child's appointment is after October 31, please call and tell me. 

2.) Physical done at school by the school physician.  Dr. Fred Heaton preforms the boys' physicals and Dr. Lunda Weaver preforms the girls' physicals.  The dates of physicals can be found on the school calendar.  You will be sent a notice when your child is scheduled and you may be present for the physical.   This is only a screening exam and if anything is deemed necessary to be further followed up you will be notified to have your family physican exam your child.


Scoliosis exam


All 6th graders in the state of Pennsylvania are examined for scoliosis (sideways curving of the spine).  This is a screening exam done during phys ed class by the school nurses.  If your child fails this screening you will receive a notice to have your child further examined by your family physician. You will receive a form in the first day of school packet explaining further this exam and with the option to exclude your child from the screening.

Vision exam


All 6th graders in the state of Pennsylvania are screened for near and far vision in the fall of the year.  If your child fails either of these screenings, you will be notified to have your child's eyes further examined by an optometrist or optician.
