7th Grade

7th Grade

The Pennsylvania Department of Health requires that all students entering 7th grade have proof of varicella immunization or chickenpox and a minimum of the first injection of the Hepatits B series.  Those students who still have to complete the remaining second and/or third Hepatitis B injection receive a form in their report card packet.  If the remaining dates of the Hepatitis B are still not received by the end of March a phone call is made to the parents.  If the dates are not in my office by April 30 , your child will be excluded from school until these requirements are metIf you have any questions or concerns please email or call me.

Dental Exams

All 7th grade students in the state of Pennsylvania are required to have a dental exam.  The exam can be preformed one year prior to 7th grade and anytime during 7th grade.

This can be accomplished in two different ways:

1.)  A dental exam form filled out by your private dentist and returned to the school by December 1.  If your child's dental exam is scheduled for after December 1, please call and give me the date.

2.)  An exam can be done at school by our school dentist Dr. Murray Smith.  You can check the school calendar for the date of these exams.  You will receive a notice when your child is scheduled for their exam and you are welcome to attend.   This is only a screening exam so if any corrections are necessary you will have to take your child to your family dentist..

Forms

Dental and physical forms can be accessed at

Scoliosis exam

All 7th graders in the state of Pennsylvania are examined for scoliosis (sideways curving of the spine).  This is a screening exam done during phys ed class by the school nurses.  If your child fails this screening you will receive a notice to have your child further examined by your family physician. You will receive a form in the first day of school packet explaining further this exam and with the option to exclude your child from the screening.

Vision exams

All 7th graders in the state of Pennsylvania are screened for near and far vision in the fall of the year.  If your child fails either of these screenings, you will be notified to have your child's eyes further examined by an optometrist or optician.
