Please keep your school nurse updated on your child's health status, such as surgeries, medications, illnesses/diseases, new diagnoses, immunizations, and/or other conditions that may affect your child's school performance.
Please make sure the back of the emergency card is completed and signed.
Certain health conditions may require an IHP (Individual Health Plan) for the safety and well-being of your child. The school nurse will contact you if this is necessary.
